If you have a confirmed physical disability and have a valid Ontario Health Card, you may be considered for an ADP funded mobility device.
To qualify, you must:
Be an Ontario resident
Have a valid Ontario health card
Have a disability requiring the equipment or supplies for six months or longer
ADP does not consider your income.

If you feel that you or someone you know would benefit from health care support at home, at school or in the community, Ontario Health atHome (OHH, previously the CCAC, LHIN, HCCSS) can open the door to a world of options and opportunities.
Ontario residents with a valid Ontario Health Insurance Plan (OHIP) card are eligible to receive an assessment completed by a care coordinator to further determine eligibility and access a wide-range of home and community care supports. Depending on the services or supports that best fit individual patient needs, there are some additional eligibility requirements depending on the program, which our staff will discuss in detail with you.

75% of an ADP Eligible Mobility Aid
full amount of an ADP Authorizers Assessment Fee, prices vary
monthly Benefits payments directly to ODSP member to assist with on-going expenses

Direct Billing to Private Insurance Plans
Ontario accessible parking permits (APPs) can be issued to a person or to a company vehicle used to transport people with a disability.
An accessible parking permit holder is the person who has the eligible health condition and will be named on the accessible parking permit.
To apply for an Ontario accessible parking permit, you must have:
a health condition that meets the eligible health requirements.
Authorization from a regulated healthcare practitioner.
You must clearly display your permit with the number and expiry date visible on your vehicle’s sun visor or dashboard to park in an accessible parking space. The permit holder must be either a passenger or driver of the vehicle using the accessible parking space.
Only the person named on the permit can use an accessible parking space. If you use the permit without the permit holder in the vehicle, you could be fined up to $5,000 and the permit may be seized.
